Where is your office and clinic located?

We are located east of Miller Auto Plaza and we are next to GCO Carpet outlet on West St. Germain Street. If you need directions please CLICK HERE for a map and driving directions. We are easily accessible to the St. Cloud Metro Bus routes.

What are your office hours?

Office appointments are scheduled between 7:00 a.m. to 7:00 p.m. Monday through Thursday and 7:00 a.m. to 5:00 p.m. on Friday.

Home Health Care appointments follow the same hours, but may be scheduled on evenings and weekends based on medical necessity.

How can I access services at SPOT Rehabilitation?

Anyone can refer someone for any program at SPOT Rehabilitation. A physician must order the service but individuals may request the order. CLICK HERE for a form that you can fill out and take to your physician for signature. The best way to communicate an order is to ask the physician to fax the order to our office at (320) 259-5707. Usually we will call you the same day as we receive the order to schedule an appointment.

How are appointments arranged?

We will make every effort to schedule your appointments at your convenience. If you are receiving more than one service, they can usually be scheduled one after the other. When you will be receiving services for a period of time, we try to make your appointments at consistent times each week. You may telephone our intake/schedule coordinators for an appointment or you may e-mail at spot@spot-rehab.com.

Will my health insurance help pay for services provided by SPOT Rehabilitation?

A health insurance plan is a contract between the patient and the insurance company. We verify insurance coverage before providing service so that you know what service is covered by your plan. As a courtesy, we will bill your insurance plan. We have established provider agreements with Medicare, Blue Cross Blue Shield, Medica, Health Partners, UCare Minnesota, Select Care, Minnesota Medical Assistance. We have also worked with many other insurance plans, including auto insurance and workers compensation.
