Will my health insurance help pay for services provided by SPOT Rehabilitation?
A health insurance plan is a contract between the patient and the insurance company. We verify insurance coverage before providing service so that you know what service is covered by your plan. As a courtesy, we will bill your insurance plan. We have established provider agreements with Medicare, Blue Cross Blue Shield, Medica, Health Partners, UCare Minnesota, Select Care and Minnesota Medical Assistance. We have also worked with many other insurance plans, including auto insurance and workers compensation.
How are appointments arranged?
We will make every effort to schedule your appointments at your convenience. If you are receiving more than one service, they can usually be scheduled one after the other. When you will be receiving services for a period of time, we try to make your appointments at consistent times each week. How can I access services at SPOT Rehabilitation & Home Health Care?
Anyone can refer someone for any program at SPOT Rehabilitation & Home Health Care. A physician must order the service but individuals may request the order. The best way to communicate an order is to ask the physician to fax the order to our office at 320-259-5707. Usually we will call you the same day as we receive the order to schedule an appointment.
