Frequently Asked Questions

A health insurance plan is a contract between the patient and the insurance company. We can assist to verify insurance coverage before providing service so that you know what service is covered by your plan. As a courtesy, we will bill your insurance plan. We have established provider agreements with Medicare, Blue Cross Blue Shield, Medica, Veteran’s Administration plans: Tri-West and Champa VA, Health Partners, Ucare Minnesota, Select Care and Minnesota Medical Assistance. We have also worked with many other insurance plans, including Auto Insurance and Worker’s Compensation.

We will make every effort to schedule your appointments at a time that is convenient to you.  If you are receiving more than one service, they can usually be scheduled one after the other. When you will be receiving services for a period of time, we try to make your appointments at consistent times each week. In order to receive SPOT Rehabilitation or SPOT Home Care services  the necessary requirement is needed;

  1. An order from the physician of what services are being ordered and faxed to SPOT at 320-259-5707.     Generally we are able to call you the same day as we receive the order to schedule either the Rehabilitation Outpatient appointment or when Home Care will be coming for their visit.

Office appointments are scheduled between 7 a.m. to 6 p.m., Monday through Thursday, and 7 a.m. to 5 p.m., on Friday.

Home health care appointments are based on medical necessity, generally 7am-6pm Monday-Thursday and 7am-5pm on Fridays, but may be scheduled on evening and weekends as medically necessary.

We are located East of Miller Auto Plaza. We are easily accessible to the St Cloud Metro Bus routes.  We have parking in front of our clinic that allows for ease to come and go for appointments.
