SPOT Rehabilitation & Home Health Care is the home health care agency of choice when you need rehabilitation services following hospitalization for an injury or illness. Quality nursing, Speech pathology,  Physical therapy, and Occupational therapy services are available. The professional staff has advanced training to provide the skilled medical and emotional support that you need to maintain independence in your home.

SPOT Rehabilitation & Home Health Care is Medicare certified and a Minnesota Licensed Class A Home Care Provider for nursing, physical therapy, occupational therapy and speech pathology. This allows skilled nursing and therapy services to be provided in the home when an individual is homebound or when the need for therapy is most appropriately provided in the home. It is most important to provide therapy when and where the individual is most able to benefit from the service.

  • Nursing
    SPOT Home Care provides Skilled Nursing to ensure professional medical oversight and management in your home. The Nurse will assess and determine a comprehensive care plan based on the identified care needs. The nurse maintains communication with your physician for enhanced care.
  • Speech therapy
    Speech Therapy provides expertise and interventions for difficulties with swallowing and communication.
  • Physical therapy
    Physical Therapy addresses treatment techniques to promote the ability to move, balance, reduce pain, restore function and prevent disability. Home Health PT promotes safety in the home in order to prevent falls.
  • Occupational therapy
    Occupational Therapy addresses the skills needed to complete the activities of daily living. Treatment may be focused on therapeutic exercise. ADL retraining, pain management, medical equipment needs, wheelchair positioning, home safety and accessibility. 
  • Social Work
    Social Work is available to assist one navigate the many life changes that may occur by providing counseling, assistance and recommending community resources.
  • Home health aide
    The home health aide is available to individuals who may need one on one personal cares of Bathing, dressing, grooming, and assistance with exercise programs.

  • Recent hospitilization
  • A change in medical condition or change of functional ability
  • Multiple medications or treatment changes
  • History of falls or unsteady walking and balance issues
  • New or long term illness or injury
  • Persistent pain that interferes with function

Referrals may be made by a physician, family member, or friend. Self referrals are accepted. Once the referral is made, the patient’s physician will be contacted for orders and a plan for services will be developed with physician oversight.

Payment for services may be made by Medicare, private insurance, self-pay, Medical Assistance, veterans administration, or a county waivered service.
