TELEHEALTH (Outpatient and Home Care)
What is Telehealth?
Telehealth is like having a therapy appointment through video chat instead of going to the office. It lets you talk to a healthcare professional from the comfort of your own home using your computer, smartphone, tablet or other devices. It allows you to have therapy without having to leave the house. It’s convenient, saves time and money on travel, and can help you get the care you need when you need it.

Call SPOT Rehabilitation and Home Care number at 320-259-4151 and speak to one of our clinic or home care schedulers.
What’s next:
They will assist you as to whether telehealth or in person would be most beneficial for needed services and schedule you as appropriate.

Indications for Home Care include:
- Recent hospitalization
- A change in medical condition or change of functional ability
- Multiple medications or treatment changes
- History of falls or unsteady walking and balance issues
- New or long-term illness or injury
- Persistent pain that interferes with function
- Increase in symptoms of Dementia
Referrals may be made by a physician, family member, or friend. Self-referrals are accepted. Once the referral is made, the patient’s physician will be contacted for orders and a plan for services will be developed with physician oversight.
Home Health Care is available in your home setting AND Telehealth, if patient desired.
Payment for services may be made by Medicare, Private Insurance, Self-Pay, Medical Assistance, Veterans Administration, or a County Waivered Service.
