SPOT ATTENDANCE AND FINANCIAL POLICIES

ATTENDANCE POLICIES:

  • Your therapy services were ordered by your physician. In order to achieve the maximum benefit from your time and effort in therapy, attending your sessions at the frequency recommended by your therapist is necessary.
  • If you need to cancel, call as soon as you know, don’t wait until your appointment time to cancel your appointment.
  • If you cancel 50% of your scheduled sessions in a given month, you will lose your permanent time on the master schedule. If you are removed from your permanent time, you will be responsible to call and schedule sessions consistently on a weekly basis. Keep in mind many of our therapists have limited availability, so if you are removed from our schedule, it will be difficult to get in for the times you would like.
  • If you do not call to cancel your appointment and you do not show for your appointment, this is deemed a “NO SHOW”. If you “no-show” two times, you will be discharged from our services and will need to seek care from a different provider.
  • A fee of $30 will be billed to you for each appointment that is deemed a “NO SHOW”. In addition to simply not calling or attending your appointment, a “NO SHOW” is determined to be arriving 15 minutes or more past your scheduled therapy time without a phone call informing our staff you are going to be late.
  • If you do not show for your initial appointment with us, you will be charged a $30 no-show fee, and we will not reschedule your appointment. You will need to seek care from a different provider.
  • If your child is attending therapy, we require that you are present in our lobby at a minimum of 5 minutes before your child’s therapy session ends. If you are consistently late to pick up your child, you will be asked to remain in the lobby for all of your child’s therapy sessions.

FINANCIAL POLICIES:

  • Patients are responsible for understanding their insurance benefits. Please provide accurate and complete insurance information at the time of your visit. If your insurance changes, please notify our office with your updated coverage information prior to the date of coverage. Any charges that may occur as the result of you not providing our office with accurate insurance information will be your responsibility to pay.
  • Our staff will verify your insurance coverage as a courtesy to you, but please know that this does not guarantee payment by your insurance company.
  • Copayments are due at the time of service. You can pay for your copay via self-check-in or at our front desk when you check in for services.
  • If your insurance plan includes co-insurance or deductibles, we will be collecting a portion of this at the time of service to prevent a large balance accruing on your account. To satisfy deductibles, we will collect $70 per visit at the time of service. For co-insurance, we will collect a percentage of the $70.00 visit fee based on your specific insurance coverage. Please note that this $70 will likely only cover a portion of the services you receive each visit, and additional charges will be invoiced to you.
  • We accept cash, checks, and major credit/debit cards. We can keep card information securely on file and can charge your card on file each time you check in with your permission.
  • Invoices must be paid within 30 days. If you have an outstanding balance, you may be required to engage in a payment plan. You will be asked to meet with our billing staff to set this up.
  • If you have overpaid on your account, you will be notified, and a refund will be issued or the amount will be applied to future services, based upon your preferences.
